Liberty Nursing Center does not discriminate on the basis of race, color, national origin age, disability, age, sex or gender identity.

The facility provides appropriate and necessary auxiliary aids and services, including qualified interpreters for individuals with disabilities, and information in alternate formats, free of charge and in a timely manner, when such aids and services are necessary to ensure an equal an equal opportunity to participate to individuals with disabilities.

The facility provides language assistance services, including translated documents and oral interpretation, free of charge and in a timely manner, when such services are necessary to provide meaningful access to individuals with limited English proficiency.

These services can be accessed by contacting the facility Administrator, Social Worker or Admissions Coordinator during normal business hours.
The facility Administrator is responsible for the implementation of the services. 

If you feel you have been discriminated against relative to these services, contact the Administrator to utilize the facility grievance procedure to achieve resolution.
