Dementia Care

Coping with dementia is challenging and often frightening. At Liberty Nursing Center of Fremont, we understand the complexities of memory impairment and the specialized care needed to create a safe, comfortable and supportive environment for people dealing with every stage of dementia and memory loss.

Care tailored to each resident’s needs
We offer three specific units dedicated to particular stages of dementia. Residents are assessed to ensure they are placed in the most appropriate setting for their needs. Our comfortable, home-like environment includes a security system that allows access for family and friends while keeping residents safe from wandering. Our dementia units are:

Iva M. Sprunk Hall – This 16-bed unit is for those with mild to moderate memory loss who area able to walk and feed themselves most of the time, but may need some assistance with other daily tasks and caring for themselves.

Criteria includes: 
   Mild to moderate memory loss
   Doesn’t require extensive medical care
   Able to walk most of the time
   Able to manage some self care with assistance
    Able to feed self most of the time 
   Can follow instructions related to simple tasks
   Participates in unit activities
   Irreversible dementia not related to drugs, alcohol
     or mental illness
   Exhibits personality and emotional changes such as wandering,
     agitation, anxiety, social isolation, delusions and depression
   Behaviors that are able to be controlled through
     non-medical/medical interventions

Thomas B. Stahl Hall – This 14-bed unit is for those with moderate to severe memory loss who need more assistance with walking, eating and daily activities.

Criteria includes: 
   Moderate to severe memory loss
    Doesn’t require extensive medical care
   Able to ambulate with one assist
   Able to manage some self care with assistance
   Able to feed self with assistance
   Can follow simple instructions with cueing
   Benefits from activities whether passive or active participation
    Irreversible dementia not related to drugs, alcohol
     or mental illness
    Exhibits personality and emotional changes such as wandering,
     agitation, anxiety, social isolation, delusions and depression
   Behaviors that are able to be controlled through
     non- medical/medical interventions

Gracie’s Place – This unit has 17 beds and is for those with advanced memory loss who need extensive care and assistance.

Criteria includes: 
   Moderate to severe memory loss
   Benefit from structured activity
     programming/living environment
   Irreversible dementia not related to drugs, alcohol
     or mental illness
   May need extensive assistance with ambulation/non-ambulatory
    May require extensive assistance with activities of daily
     living; dependent
   May be dependent with feeding
   Behaviors that are able to be controlled through
     non-medical/medical interventions

We understand that each resident’s needs can change as dementia progresses, so residents are moved if they no longer meet the criteria of their unit. 

Activity programs enrich lives
Our philosophy of care focuses on activity programs to keep residents engaged and reduce the need for medication. Our stimulating activities provide interaction for residents and help them structure their time while enhancing their well being by adding purpose and meaning to everyday life.

A caring and compassionate staff
Our state-tested nurse aides are specifically trained to deal with the needs and behaviors of residents with dementia. Providing gentle and supportive care, residents receive the assistance they need while maintaining their dignity and respect. A neuropsychologist and psychiatrist are also available to provide services and consultations, as needed, to help develop the best plan of care for individual residents.
